To save time, you can print out these forms, complete them at home, and bring them with you to your appointment. Please be prepared to provide this information to our office along with your current medical insurance information.
If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

Dr. Brotzman does not treat injuries of the spine or cases involved in any form of litigation.
Once your test has been scheduled we typically receive your results anywhere from 2-5 business days after your test. Please allow us time to receive and review your results. If your test was ordered STAT we should notify you of results within 48 hours. If something emergent is found upon review of your studies we will notify you as soon as possible to make a follow-up appointment.
If you have a follow-up visit already scheduled please keep that appointment so that you may review your findings with the physician, we will still call you and inform you of your results. If you do not hear from our office within 3 days of having your study, please give us a call. In order to expedite your results make sure that we have your most current telephone number and address.
What is your Prescription Refill Policy?
If you would like a prescription refilled please ask your pharmacy to fax the refill request to our office at 512-977-0020. If you would like to contact our office for you refill please be aware that we may not be able to return your prescription refill call until the end of the business day.
We will receive refill requests from 8:00 A.M. – 5:00 P.M. Monday thru Friday. Any attempt made on the weekend will not be reviewed until the next business day. Please avoid waiting until Friday afternoon to request a refill. Call your pharmacist during business hours no later than Thursday afternoon and ask them to please fax your request in so that we may have ample time to respond.
